1) I understand that it is mandatory for COLABS applicants to obtain Consent of Acceptance 　from one prospective academic advisor beforehand as it is a research-oriented program. * 2) I understand that obtaining the above-mentioned consent does not guarantee acceptance to the COLABS program.* 3) I understand that failure to submit any of the application materials to TAO (incomplete applications) 　shall result in disqualification from further screening procedures, regardless of whether the above-mentioned consent 　was obtained. * 4) I promise that all information I provide in my application and related materials are true and correct. * 5) I promise that both during and after my participation in the COLABS program at Tohoku University, 　I will be enrolled as　a full-time master's degree seeking student with excellent grades at my home university. *
